Carer Coordinator

JAM CARE is recruiting for experienced Carer Coordinators to join our growing team at our JAM HQ.

​

Care Coordinator Responsibilities

​

  • Coordinate production of the monthly rotas. Manage and resolve rota queries, challenges and issues. Provide “on-call” support to care packages.  Finding smart solutions to staffing challenges and shift cover gaps when required.

  • Liaise with and coordinate staff, clients and key health stakeholders (District Nurses, Social workers, GPs, OT’s, Hospital etc.) to ensure delivery of great care to our clients.  Support JAM’s Nursing Team to ensure new staff are clinically competent.

  • Maintain comprehensive and up-to-date records including care plans, client and staff file notes, training files and risk assessments and help provide coaching and training.

  • Help plan new clients care packages working with JAM to create an amazing service.  Recruit and onboard new carers to support new care packages.

  • Provide care and support in the new client’s home for 2-4 weeks.  Identify and solve problems and constantly feedback improvement ideas to the Head Office, ensuring delivery of a high-quality care service by JAM.

  • Be committed to continuous learning and training. You will become an elite and highly skilled Complex Care Provider, able to be deployed across any new care packages, ensuring an amazing introduction to JAM CARE for new clients.

  • You will be caring for and supporting clients in their own homes and supporting their families and loved ones. The role involves working with vulnerable adults who require help and support which could be one or a combination of the following: clinical, practical or mental.

  • You must follow the client’s Care Plan and follow the instructions of the Nurse Manager and will work in conjunction with colleagues, JAM Care Nurse Manager and the wider Multi-Disciplinary Team (District Nurse, Occupational Therapist, GP etc.).

  • At times you may be expected to accompany your client to visits and appointments e.g. GP, hospital, days out, shopping trips etc. and may be asked to care for your clients during any hospital stays.  Brilliant carers are those who really know their clients.  This comes from spending time with them, talking to them, listening to them and practicing key tasks.

  • You will participate in training and development as required in the interests of our overall service and to meet statutory requirements.

 

Care Coordinator requirements

​

  • A valid Right to Work in the UK

  • UK Driving licence + access to a car (or a way to get to work)

  • Previous Carer experience, or transferable skills

  • To live in the area and be willing to travel to the homes of our clients

  • Be a great communicator and have excellent relationship skills with an ability to communicate on all levels
